PAYMENT SCHEDULE will be advised by your agent:
Non-refundable deposit at time of booking. No booking is considered confirmed without a deposit.
Optional Travel Insurance is highly recommended at time of booking

These standard terms may be overwritten on our invoice by more restrictive requirements based on the package booked. If airfare is included in the package, it is subject to availability until your deposit is received. Should the rate this package is based on no longer be available, a surcharge will apply.
Preferred form of payment is Interac eTransfer (within Canada only), bank drafts made out to Fly & Sea Dive Adventures, wire transfers or Visa/Mastercard/American Express. It is mandatory that the payment schedule as outlined by your agent, be observed. Should you fail to comply, this could result in cancellation of your reservation with the applicable penalties. All payments are non-refundable and non-transferable.
All cancellations are required in writing. Any changes made to the package, when possible, are subject to a fee. The following per person charges will apply in the event of cancellation prior to tour commencement:
Up to 100 days prior to departure
100 days or less prior to departure
100% of total cost

Due to strict cancellation penalties comprehensive travel insurance is highly recommended. We represent Manulife/CSA Travel Protection, one of the most reputable insurance providers in North America. A travel insurance quote is available on our website, or by contacting our office. Our agents will provide you with a quote specific to your trip. Insurance premiums once paid are 100% non-refundable. Please note: Dive accident insurance is mandatory with many dive operators. Manulife/CSA’s emergency medical coverage includes diving accidents.
NO REFUNDS AFTER DEPARTURE:
Failure to make final payment shall constitute a cancellation and applicable penalties will apply. Once documents are issued or after travel has commenced, changes may not be possible. If a change is possible, it will be subject to a fee.
A valid passport is required and should be valid for at least six months beyond the date of your expected departure. Please check your passport now to avoid any last minute rush. Please ensure you provide us with your legal name as it appears on your passport. It is your responsibility to check your travel documents and visa requirements for the country you will be visiting.
The below liability waiver is required to be completed for every trip, prior to the delivery of travel documents.
Dive operators require a signed liability waiver form, which may need to be returned to us prior to the trip. A self-declaration medical form may also be required (a copy can be requested from us). Please review the medical form prior to your trip to determine if a doctor’s permission to dive is required. Do not send the medical form back to us, take it with you on your trip.
Fly & Sea Dive Adventures does not take responsibility for any changes and adjustments made by our suppliers, including airline over bookings and schedule changes, which could alter the overall package and cost. Luggage restrictions may cause surcharges by airlines and are not included in the package price.
All dive sites are subject to weather conditions and no refunds are available for missed dives due to adverse weather conditions.
We require a legible photocopy of the main pages of your passport with this application.
